Nevada has been awarded a nearly $10 million grant to expand Community Based Services designed to get institutionalized people out of long-term care and into group homes.
The six-year grant was awarded by the Centers for Medicare and Medicaid.
The grant is to continue development of the support systems needed to let those persons live in the least restrictive environment.
During the next six years, the plan is to transfer 524 people into community based care.
The money will be used to strengthen outreach and community integration services, build mechanisms to use the savings produced by moving people out of nursing homes to expand funding for community based services.
Of the total, $5.5 million will help support costs of Medicaid home and community based services. Some $1.9 million will provide transition services, helping to prepare households for individuals moving out of institutional care.
A $2.3 million grant is for administrative functions, bringing the total to $9.7 million.
