Silver State Accountable Care Organization recently named Carson Tahoe Health’s Medical Group the winner in four categories listed in last year’s “Close the Gap Contest.”
Silver State ACO works with Medicare and local providers, such as CTMG to improve patient health outcomes and conserve resources for Medicare through detection, treatment, and management conditions that could lead to complications.
The organization defined seven key metrics for the contest, including breast cancer screening, colorectal screening, tobacco use and cessation, influenza immunizations, fall risk screening, controlling high blood pressure and depression screening.
Of these seven metrics, CTMG closed the most gaps in breast cancer screening, colorectal screening, tobacco use and cessation, and depression screening.
This recognition comes off the heels of the multi-agency Community Health Needs Assessment, where community members identified several pressing health issues, including mental and emotional well-being.
